The correlation between skin-prick tests, nonspecific bronchoprovocation test and obstruction in small airways

Abstract

In this study we confirm that isolated finding of the obstruction in small airways is frequently a sign of tracheobronchial hyperactivity. Testing was performed in 40 subject in whom obstruction in small airways was diagnosed. For testing we used parameters of spirometry, flow volume curves, and body pletysmography. In these subjects the non specific bronchoprovocation test with acetilcholin and skin prick testing with the inhalation allergens were performed. Determination of nonspecific bronchial hyperactivity by the acetilcholine inhalation showed a very large degree in 12 patients, medium to great degree in 6, medium degree in 8 and small degree in 4 patients. Only 6 patients didn't show reduction of lung function of 20%. There was statisticaly significant correlation between obstruction and hyperactivity in small airways. The results support the assumption that obstructive lung disease begins in small airways. We recommended that subjects in whom tests on flow volume curves demonstrate decreased values of flow rates at lower lung volumes, in spite of normal spirometry should be completely clinically examined, followed and cured medically while the obstruction is reversibile and theraphy helpful.
