Asthma-related quality of life in the GOAL study: Baseline results

Abstract

Rationale: The purpose of the GOAL (Gaining Optimal Asthma ControL) study is to determine whether asthma control, assessed by a composite measure of the GINA/NIH goals of asthma management, is achievable. Design: GOAL is a 52-week, multicenter, randomised, double-blind, parallel-group, step-up comparison of asthma control achieved with salmeterol/fluticasone propionate combination compared with fluticasone propionate alone in patients stratified by prior inhaled corticosteroid (ICS) use. The Asthma Quality of Life Questionnaire (AQLQ) was used at baseline in those countries where a validated translation was available. On a scale of 1 to 7, higher scores indicate better quality of life. Results: Following a four-week run-in of usual therapy, over 50% of 3415 [dsquote]not well controlled[dsquote] patients completed a baseline AQLQ. The mean overall score was 4.5 (SD±]1.4); 7% indicated little or no impairment in asthma-related quality of life (>=6.0), 85% had moderate impairment (>3 but <6) and 8% reported severe impairment (<3). Conclusion: The wide range of baseline AQLQ scores suggests some patients learn to cope with their disease and lower their expectations about control while others are severely affected. Similar mean overall AQLQ scores in the three strata suggest the impact of uncontrolled asthma is similar regardless of baseline treatment.
