Abstract

In Novosibirsk region TB-notification rate is higher then all over Russia, every year this rate is increasing, in 2001 it was 137.4 per 100000 inhabitants).The extrapulmonary TB is increasing too. The aim of our work was to study the epidemiological aspects of tuberculous meningitis (TBM) in Novosibirsk region during the last three years. There were 5 cases of TBM in 1999, 10 in 2000 and 16 in 2001, 31 in total. There were 26 male and 5 female, in 1999 and 2000 only male, in 2001 5 female and 11 male .2 cases of TBM occurred in children in 2000 and 2001, both boys before 1 year old, contact with TB mothers. The average age among adults was 40.2 years old. In 1999 the prevailing age group was 40-50, in 2000- 30-40 years group, in 2001- 20-30 years group. The prevailing age group in pulmonary TB is still 30-40 years old. There were no migrants, the proportion of city and rural inhabitants was about 1:1. 35.7% of patients were unemployed, 41.9% were convicted formerly, 42.8% were alcoholics, 67.8% smoked, 10.7% used intravenous drugs. No HIV- infected patients were notified. In 85.7% cases TBM complicated by pulmonary TB. In 28.6 % cases TBM developed while using antituberculosis drugs irregularly. The lethal outcomes are very frequent –44.8%.
Conclusion. Number of cases of TBM is markedly increasing, newborns TBM appeared. TBM in adults is becoming [dsquote]younger[dsquote], it usually occurs in socially unadapted patients, convicted by pulmonary TB, sometimes while using antituberculosis drugs. TBM is still the disease with a high mortality level.
