Role of neutrophils in airway inflammation in children with chronic bronchiolitis

Abstract

Chronic bronchiolitis (CB) is characterized by irreversible airway obstruction and chronic airway inflammation with neutrophils involvement and release of different inflammatory mediators including interleukin-8 (IL-8). The aim of the study was to investigate bronchoalveolar lavage (BAL) cell profile and levels of IL-8 in blood serum and BALF in children with CB depending of severity of disease and on the degree of bronchial obstruction. We examined 57 children with CB (mean age 11,5±]0,4 years) in periods of marked clinical exacerbation (E) and relative clinical remission (R). Sera from 12 healthy children age 10-12 years were used as controls. Ten adult healthy controls were included for BAL procedure. Concentrations of IL-8 were measured by ELISA. CB was characterized by a high median ratio of neutrophils (from 31,7 to 70,4 %), eosinophils (from 2,1 to 7,9%) and lymphocytes (from 7,6 to 16,5%) in both E and R in all patients compared with healthy controls. We revealed high concentration of IL-8 in BALF (from 880,6 to 1838,3 pg/ml) and serum (from 42,0 to 88,6 pg/ml) in both E and R in all patients compared with healthy controls. Furthermore, BALF concentrations of IL-8 were 10 to 40 times higher as compared to its serum levels. High percentage of neutrophils and levels of IL-8 were revealed especially in patients with severe CB. Data from all CB patients demonstrated correlations between neutrophils (%) and FEV1 (r=-0,845, p<001), between neutrophils (%) and IL-8 (r=0,748; p<0,001). We conclude that neutrophils are dominant type of cells in low airway in children with chronic bronchiolitis.
