Abstract
Serum thyroid hormone levels drop during illness. This decrease in serum thyroid hormone levels is seen in sepsis, surgery, myocardial infarction, bone marrow transplantation and, in fact, probably any severe illness.
The aim of this study was to estimate the influence of severe respiratory failure on blood serum thyroid hormone concentrations in a group of patients admitted to the Respiratory Intensive Care Unit of our Institution. For this end blood samples for serum thyroid hormone level assay were collected at admission in 25 patients (13 males; mean age 69.6±] 11.1) with acute respiratory failure needing mechanical ventilation (iron lung and/or noninvasive or invasive positive pressure ventilation).
The principal results were: 1) in the majority of patients FT3 levels were wherein the lowest normal range; 2) in 30% of patients FT3 was very low and these patients had a poor outcome; 3) FT4 levels were low in only one patient; 4) TSH levels were pathologically low in 50% of all patients.
In conclusion, this preliminary data indicate that the low levels of thyroid function, observed frequently in this sample of patients, were correlated with the severity of respiratory failure and outcome. We are now evaluating if the sostitutive triiodothyronine therapy could improve the outcome of this kind of patients.
