Long-term haemodynamic response in patients with pulmonary hypertension inhaling iloprost (AIR-2 STUDY)

Abstract Objectives To investigate the long-term hemodynamic (HD) response in pulmonary hypertension patients treated with inhaled iloprost for up to 24 months. Design Open-label multicenter study of inhaled iloprost in primary (PPH) or certain forms of secondary pulmonary hypertension (SPH). Methods Pre and post-inhalation HD examinations were performed at baseline (BL) and after up to 24 months. Post-inhalation HD were measured after a single inhaled iloprost dose of ~2.8 μg. The long-term median daily aerosolized dose was100 μg (total inhaled dose ~24 μg) divided into 6 inhalations. Results Out of the initially included 63 patients, HD examinations were performed in 33 patients (24 PPH and 9 SPH). 2-year pre-inhalation HD of these patients compared with BL showed a trend to improve: Mean pulmonary arterial pressure (mPAP) decreased by 2.2 ± 2.2 mmHg (mean ± SEM) [(total n=33), PPH (n=24) -2.3 ± 3.0; SPH (n=9) -1.7 ± 1.8], pulmonary vascular resistance (PVR) decreased by 126.1 ± 74.4 dyn×s×cm-5 [(total n=28), PPH (n=22) -179.9 ± 83.7; SPH (n=9) 70.8 ± 147.7], cardiac output (CO) increased by 0.14 ± 0.21 l/min/m² [(total n=33), PPH (n=24) 0.33 ± 0.24; SPH (n=9) -0.35 ± 0.39]. PPH pre-inhalation PVR was statistically significant (p=0.047). Overall and for PPH, post-inhalation HD (mPAP, PVR, CO) were significantly improved compared to BL. Conclusions Inhaled iloprost therapy results in long-term stabilization of pre-inhalation HD in PPH and SPH patients. Overall and for the PPH subgroup, significant improvement of post-inhalation HD was confirmed.
