Association between portopulmonary hypertension and hepatopulmonary syndrome in cirrhotics, candidates to hepatic transplantation

Abstract

Introduction: The vascular disorders presented in patients with chronic liver disease are variable, like hepatopulmonary syndrome with the predominance of intrapulmonary vascular dilatations and portopulmonary hypertension with elevated pressure in pulmonary arterial system. This two disorders can be presented in a same patient, but there are so few cases related.
Aim: to determine the prevalence of these two disorders in a same patient, in a group of cirrhotic patients who will be submitted to hepatic transplantation.
Methods: In a group of 112 patients with the diagnosis of chronic liver disease who will be submitted to hepatic transplantation. From this group all patients were submitted to Chest X-Ray, arterial blood gases analysis, Alveolar-arterial gradient(AAG) calculated, Doppler echorcardiography with mensuration of tricuspid regurgitation jet and systolic pulmonary blood pressure determination and intrapulmonary shunt determination by the infusion of saline solution in a brachial vein. We consider pulmonary hypertension in that cases that the tricuspideal regurgitation was superior to 40 mmHg, and hepatopulmonary syndrome in the cases that the AAG was superior to 15mmHg with positive pulmonary shunt confirmed by the Doppler echocardiography.
Results: We found the two disorders presented in same patients in 13 cases (11,6%) of this studied group.
Conclusion: The association between the two disorders in this group has an elevated frequency. Their coexistences need to be more evaluated in cirrhotic patients.
