Comparative characteristics of bone metabolism in the smokers and non-smokers with COPD

Abstract

COPD is associated with extrapulmonary effects; one of them is osteopenic syndrome. Cigarette smoking is by far the most important risk factor for COPD and smoking also negative influences on bone metabolism.
The aim of study was to assess the effects of the smoking on bone metabolism in patients with COPD. All COPD patients were divided into 2 groups: 63,3 % - smokers, 36,7 % - non-smokers.
Correlation analyses revealed negative relation between indices of total park-years and serum alpha1-antitrypsin (r=-0,53, p<0,05) and positive correlation between indices of park-years and elastasa activity (r=0,66, p<0,01). At last our studies was showed that body mass index (BMI) is predictor of osteopenic complication in the patients with COPD. It was established the BMI in COPD was lower in the smokers than non-smokers (p<0,01). We revealed a significant correlation between of level bone formation marker osteocalcin (OC), bone mineral density scores and indices of park-years (r=-0,71, p<0,01; r=-0,69, p<0,01, respectively). The smoked patients have expressed decrease of OC (21,3±1,7 ng/ml) in comparison with non-smokers (40,27±2,3 ng/ml, p<0,01). Increased of marker bone resorbtion CrossLaps (CL) was found in both groups of patients. We did not found significant different of CL level between the smokers and non-smokers. The dysfunction of bone remodeling in non-smoking patients with COPD is connected with bone resorbtion predominance.
The results suggest that smoking (i.e., total pack-years) is supplementary negative affect on the bone metabolism, mainly depressing the formation of bone.
