Abstract

Nutritional abnormalities have been suggested to affect the functional status and peripheral muscle (PM) characteristics in COPD. However, limited data is available about PM histology, and exercise capacity between underweight (UW), and normal weight (NW) COPD patients. We studied 9 UW (BMI<21 kg/m²; FEV1 0.86±0.20), and 11 NW (BMI>21 kg/m²; FEV1 1.0±0.24) patients. All patients underwent a PFT, 6MWD, and trascutaneous biopsy of the vastus lateralis muscle. The PM biopsies were analyzed for fiber-types proportion, area of the fibers (CSA), and capillarity. No significant differences were observed in the following data: 6MWD was 379±72 m in NW and 378±96 in UW. Fiber types I, IIa and IIb were in NW: 37±12, 41±7 and 22±11%, and 37±8, 38±9 and 25±7% in UW. Mean CSA was also similar: 5268±1231 µm² in NW and 5090±1438 in UW. Capillaries/mm² were 293±46 in NW vs.386±43 in UW. Capillary contacts/fiber CSA were in NW, for types I, IIa and IIb: 0.75±0.11, 0.79±0.21 and 0.85±0.31 vs. 0.69±0.15, 0.79±0.12 and 0.88±0.19 in UW. We conclude that PM characteristics and exercise performance are similar between NW and UW COPD patients. These data could help explain the lack of changes reported on muscle dysfunction and exercise performance with nutritional intervention in COPD.
