Relationship between loss of body weight and impairment of tissue oxygenation in patients with pulmonary emphysema

Abstract

Purpose: Loss of body weight is one of the risk factors in patients with pulmonary emphysema, and tissue oxygenation is impaired in patients with severe pulmonary emphysema. This study investigated the relationship between body mass index (BMI) and tissue oxygenation with severe pulmonary emphysema.
Methods: Twenty-one patients with stable pulmonary emphysema were enrolled in this study. All patients were male with age of 65±7 years-old, height 161±6 cm, weight 46±6 kg, and BMI 17.8±2.3 kg/m2. FEV1.0% was 27±8%, RV/TLC 55±7%, %DLco 61±24%, Pao2 69.5±12.8 mmHg, and Paco2 44.1±7.2 mmHg.
Pulmonary hemodynamics were examined using right heart catheterization techniques under three different breathing conditions; room air breathing at rest, 100% O2 breathing at rest, and 100% O2 breathing with forced rapid ventilation (40/min). Arterial oxygen content (Cao2) and mixed venous oxygen content (Cvo2) were obtained, and tissue oxygenation (Vo2) under three breathing patterns was calculated as follows; Vo2 = Cardiac index (CI) x (Cao2 - Cvo2).
Results: There was no clear relationship between Vo2 and BMI under room air breathing at rest. However, BMI was significantly lower in patients whose increase in Vo2 was less when breathing pattern was changed from normal to rapid breathing under100% O2.
Conclusion: Loss of body weight may further deteriorate the condition of patients with pulmonary emphysema by impairment of tissue oxygenation (Vo2) during exercise.
