Abstract

Lungs are the most frequently affected organ in sarcoidosis and their progressive fibrosis can result in respiratory impairment. Recently, some studies showed Th1/Th2 balance shift towards Th1 immunity in patients with active sarcoidosis.
Dendritic cells (DCs) are recognized as essential regulators of immune system. Two lineages of DCs have been described: blood myeloid and lymphoid DCs. The first are associated with Th1-type response, while the latter with Th-2. To assess importance of DCs in sarcoidosis we estimated blood myeloid (BDCA-1+) and lymphoid (BDCA-2+) DCs both in peripheral blood (PB) and bronchoalveolar lavage fluid (BALF). The study population consisted of 22 patients with sarcoidosis, divided into three groups, according to the type of chest radiograph.
We observed, that percentage of myeloid DCs in BALF from patients with type II and III chest radiograph sarcoidosis was significantly higher compared to patients with type I (p<0.05). The BDCA-1/2 ratio in BALF in type III was higher than in type I (p<0.05). As type II and III are associated with lung fibrosis and Th2 type cytokines are important mediators of fibrosis, these data may indicate, that DCs play protective role in course of this disease.
