Hypercalcemia and low parathormone levels in active pulmonary tuberculosis

Abstract

The aim of this study was to evaluate calcium (Ca) metabolism in pulmonary tuberculosis (PTB). For this reason serum Ca, and parathormone (PTH) levels were measured in 45 patients with active PTB, serum phosphorus (P) and magnesium (Mg) levels were measured in 35 of these patients. Serum Ca, PTH, P and Mg levels were measured in 21 healthy control subjects.
Results:Five of the 45 patients (11%) had serum corrected Ca levels above the reference range for our laboratory. Ten of the patients with PTB (22%) had PTH levels below the reference range. A negative correlation existed between serum PTH levels and the radiographic extent of disease (r=-0.49, p=0.001). A positive correlation existed between serum corrected Ca levels and radiographic extent of disease, but this was not significant (r=0.29, p=0,053). No relation was found between Mg and P levels and radiographic extent of disease. The mean corrected serum Ca level was significantly higher in patients with PTB (10,0±0,5 mg/dL) than in the control group (9,2±0,5 mg/dL,p=0.000). The mean serum PTH level was significantly lower in PTB group (17,8±8,8 pg/ml) than in the control group (47,2±16,2 pg/ml,p=0.000). There was no difference for mean serum Mg and P levels between the PTB and control group.
Conclusion: This study suggests that serum PTH levels may be depressed, this depression may be related with the radiographic extent and there is a significant abnormality in Ca metabolism in some patients with PTB.
