The mistakes in the diagnosis of the tuberculosis for children under 7 years old

Abstract

The aim of the research was to investigate the frequency and causes of the divergence between the clinical and patomorphological diagnosis of tuberculosis (TB) for children under 7 years old. 339 protocols of autopcy have been analised. There are 100 children under one year old, 149 - from 1 to 3, 90 - from 3 to 7. TB was first revealed in autopsy in 45 (13,3 %) children (under one year old - 23,0 %, from 1 to 3 - 12,1 %, from 3 to 7 - 4,4 %). From 287 children with pulmonary TB for 59 (20,6 %) children the TB was revealed in the autopsy (under one year old - 14,7 %, from 1 to 3 - 19,4 %, from 3 to 7 - 27,7 % ).The nonspecific pneumonia was diagnosed erroneously for 13 (4,5 %) children moreover the frequency of mistakes in the age groups was equally (under one year old - 4,0 % , from 1 to 3 - 4,7 %, from 3 to 7 - 4,8 %). The most often 22.7 % diagnosis didn’t coincided for children under one years old and from 1 to 3 - 32.6 %, in children from 3 to 7 - only 8,4 %. 322 (95,0 %) children died in non tuberculosis hospitals. The death was during one day for 7,4 % children, during next two days for 16,1 % and 19,6 % - after one month in hospital. Conclusion.TB was diagnosed in autopsy for 13,3 % cases. The causes of diagnosis mistakes: hord state of health becouse of the late hospitalization and insufficient to qualification of pediatrist.
