Abstract

The results of treatment of pneumothorax after the first revealing of lungs tuberculosis of 110 patients have been studied (men – 75, women - 35). 77.2% of patients’ ages were from 20 to 40. Tuberculosis of the right lung was discovered in 25.5%, of the left – in 38.1%, of both lungs – in 36.4% and spontaneous pneumothorax on the right - 38.2%, on the left – in 59.1%, on both sides – in 2.7%. In disseminated tuberculosis spontaneous pneumothorax occurred at 27.3% of patients, in infiltrate – 51,8%, in fibrocavernous – 20,9%. In 70,9% cases tuberculosis of lungs and spontaneous pneumothorax were discovered simultaneously, in 29,1% spontaneous pneumothorax developed 1-1.5 month later after the diagnostics of lung tuberculosis. Smear positive was in 67.3%, pleural liquid was smear positive by culture in 31.8%. Nonspecific bacteria (Staphylococcus aureus, Pseudomonas aerogenosa, Streptococcus and others) were found in pleura in 53.6%. 53 patients (48.2%) showed good clinical effect after puncture aspiration treatment (19) and drainage of pleura cavity (91). 18 patients (16.4%) died. Operations were made on 28 patients (25.5%): pulmonectomy on 19, pleurectomy and partial resection of the lung on 4, pleurectomy on 5. 20 (71.4%) patients showed good results after operations, lethality was in 5 cases (17.9%).
Conclusion: 66,4% patients with spread first revealed destructive tuberculosis complicated with spontaneous pnemothorax showed good results after combined therapy followed by puncture aspiration, drainage of pleura cavity and surgical treatment.
