Aspergilloma associated with active pulmonary tuberculosis: 4 cases report

Abstract Although pulmonary aspergilloma take place essentially on remaing tuberculosis cavities, the association with active pulmonary tuberculosis is exceptional accruring in non immunodeficient patients.Among 21 cases of aspergilloma listed in our departement since January 1991, 4 patients (20%) had a current active pulmonary tuberculosis. Only 2 patients had a previous history of pulmonary tuberculosis and hemoptysis was the main clinical symptom. Scan findings allowed the aspergilloma’s diagnosis in all but have shown active tuberculosis lesions only in 1 patient. In all cases, tuberculosis diagnosis was confirmed by bascilloscopy. In 2 operated patients for pulmonary aspergilloma ,evolution was favourable after tuberculosis treatement. In the third case, inspite of clinical improvement, there was no radiologic changes at the end of tuberculosis treatement. An aspergilloma occurred in the other lung 2 years later on the last patient with a chronic respiratory insufficiency who died quickly after the end of tuberculosis treatement .This relatively high frequency of this association is unusual because of aspergillus toxins known to inactive Koch bacillus profileration in non immunodeficient patients.
