Abstract

The article shows epidemiological data on cases of Mycobacterium tuberculosis and MOTT infections in a hospital containing 90 beds in the Internal Medicine Ward.
This summary includes whole 2002-year.
Detailed information we present based on history of 165 cases with tuberculosis (TB).
During this year there were 3534 hospitalisations: 2113 women (60%), 1421 men (40%). In this group were 574 (16,2%) cases of respiratory diseases. The history of TB had 149 patients (4,2%), 4 had history of M. kansasi infection and 2 were already TB diagnosed and received treatment.
17 patients (0,5%) were suspected of having active pulmonary TB mostly on base of respiratory symptoms, examination, radiographic pattern and tuberculin skin test. In order to further diagnostic and treatment, 11 (64,7%) (2 women, 9 men) patients were transferred to co-operating centre of TB management.
In the second group of 6 patients (3 women, 3 men) suspected of active TB, M. tuberculosis was confirmed in 1 case - from BAL: 1 smear positive, moreover M. gordonoe and M. kansasi were isolated in 2 other cases - from BAL: positive culture. Those patients we directed to the pulmonary outpatient clinic. 2 of our patients were first diagnosed to have TB after death. Both of those 2 cases were diagnosed due to hectic fevers and suddenly died (1 case of miliary TB diagnosed at autopsy, 1 positive culture result we’ve got after patient’s death). Those 2 cases were very difficult in diagnostic and will be presented in the article.
Conclusion: TB is still an important diagnostic and therapeutic problem in internal medicine wards despite low admission rate.
