TB of the pleura as a first manifestation of TB in the respiratory tract in prisoners

Abstract

Pleural effusion (PE) occur in 3.9% of respiratory diseases among prison population of St. Petersburg region.
During 2000-2003 years 103 patients have been admitted to our hospital for PE of unknown origin. 101 of them (98 %) were males 2 (2 %) - females aged 19 - 43 years, 88 (85.4%) younger than 30, average age 26 +/- 1.4 years.
For 85(82,5%) patients it was a first imprisonment. None of them previously had had TB history. In all the patients examination was made including lung X-Ray, bronchoscopy, sputum and effusion cytology, smears by Ziehl-Neelsen and Levenstein-Iensen cultures (LIC), needle biopsy of the pleura.
The examination didn’t help to reveal the origin of the disease in all the patients.
The main group was formed of 34 patients who underwent diagnostic videothoracoscopy (VTS) with biopsy of demonstrative lesions of the costal pleura. 69 remaining patients were the control group
In 32 patients (94.1%) solitary TB nodules at the pleural surface were revealed during VTS and confirmed after histology in 31 (96.9%) and LIC in 14(43.8%) patients. In two (5.9%) remaining patients non-specific inflammation was the reason of PE. No major complications occur.
Among all the patients in the control group the reason of PE remained unidentified, in seven (10.1%) of them lung TB on the ipsilateral side was revealed during 1 year after admission.
We observed high rate of TB revealed in young prisoners suffering with PE. It allows us to suppose that PE in such patients is the first manifestation of TB.
