Intra-breath CO-diffusion and pulmonary blood flow at rest and during exercise in pulmonary arterial hypertension and pulmonary fibrosis

Abstract

We investigated whether pulmonary blood flow (Qc) by means of the absorption of acetylene is a more sensitive and specific measure to diagnose pulmonary hypertension (PH) than diffusion of carbon monoxide. Transfer factor for carbon monoxide (TLCO) and Qc at rest and during exercise were measured in 15 patients with pulmonary arterial hypertension, 10 patients with pulmonary fibrosis without PH and 15 healthy control subjects. The healthy controls did not show a significant difference between their measured and reference values. Patients with pulmonary hypertension showed significantly lower values both at rest and during exercise, for the TLCO (rest: 54%±23 of predicted; exercise 49%±21) and Qc(rest: 52%±28 of predicted; exercise: 36%±19). Patients with pulmonary fibrosis showed comparable low values for the TLCO (rest: 57%±21; exercise 53%±24) but Qc values within normal limits (rest 116%±35;exercise 99%±27). The increase of Qc during exercise as an indirect measure of the recruitment capacity of the pulmonary vascular bed was lower in the pulmonary hypertension group (24%±29) compared to the patients with pulmonary fibrosis (85%±41) and controls (112%±32). These results show that a distinction can be made between patients with pulmonary hypertension, pulmonary fibrosis without PH and healthy subjects based on intra-breath absorption of acetylene at rest and during exercise.
