Abstract

Alpha-1-antitrypsin (AAT) deficiency is underdiagnosed in most developed countries. As much as 8,000 deficient individuals must exist in Spain, although only 340 are diagnosed. We are conducting a screening program based on determination of AAT concentrations and genotyping in samples obtained from dried blood spots (DBS). We previously described a rapid and reliable method for high throughput PIS and PIZ deficient genotype screening from DBS specimens using the high-speed, LightCycler analyzer (Roche Diagnostics, Mannheim, Germany). The screening program is conducted in patients with COPD through the collaboration of the participants in the workshop for COPD of the Spanish Society of Chest Physicians (SEPAR) with the aim to obtain 3000 samples. A total of 1,958 samples have been obtained. The results show normal concentrations in 1,758 (89.7%); low concentrations in 121 (6.2%); and insufficient sample for quantification in 79 (4%). Genotyping demonstrated 5 individuals to be PiZZ (0.2%) and 35 individuals were heterozygous MZ (1.8%). Screening is effective in identifying individuals with the deficiency. Adequate selection of candidates may increase the diagnostic yield of the program.
