Effect of HIV counseling and testing program on patients‘ adherence of isoniazid preventive therapy (IPT)

Abstract

More than a quarter of subjects, whom IPT was indicated, failed to complete it. Since 2003 in some regions of Ukraine voluntary HIV counseling and testing (VCT) program has been implemented. Before 2003 we used only voluntary HIV testing (VT). The aim was to assess the impact of VCT on patients adherence of IPT. VCT targeted to improve tuberculosis education among HIV-infected patients. In retrospective study we compared acceptance level of PPD skin testing and adherence level of IPT in patients who were notified in 2002 before implementation of VCT in Kiev and in 2003 after implementation this program. In Kiev in 2002 were notified 372 patients with HIV/AIDS, in 2003 — 841. In 2002 182 (48,9%) patients were agreed to perform PPD skin test and 70 (38,4%) from them completed it. These proportions increased significantly in 2003 after posttest counseling — 825 (98,1%) and 554 (67,2%) respectively, p<0,01. In result of PPD screening 42 patients, who didn‘t were counseled, were indicated to IPT, 17 (40,5 %) of them completed it. Among posttest counseled patients IPT was indicated for 402 patients and 292 (72,6%)of them completed it. So, proportion patients, who completed IPT, was significantly higher in case of conducting posttest counseling, p<0,05. Conclusion. VCT significantly increases acceptance level of PPD skin testing and adherence level of IPT.
