Abstract

Aim: to identify the risk factors for default in new pulmonary TB cases. The experience of the first Romanian DOTS pilot project, developed in Iasi County, Romania, showed that between 1998-2003 26 patients defaulted yearly from the first TB treatment.
Method: a comparative retrospective study of different characteristics in 2 samples of new pulmonary TB cases, with the same demographic characteristics was done.
Results: relevant differences between factors who could determine the default were: unemployment (p<0.0001), homeless (p<0.0001), the marital status -(p<0.01), low levels of education (p<0.0001), a longer time of hospitalization, other associated diseases-specially alcoholism-(p<0.0001), 62% patients defaulted in the continuation phase; 60% of the patients returned for treatment; 77.7% were finally successfully treated, 18.5% defaulted again and 3.3 %died.
Conclusions:
The key risk factors associated with default are sex male, alcoholism, unemployment and homeless.
The socio-economic characteristics had the strongest influence for default; their earlier detection and evaluations could be used to prevent the default in the future.
