Outcome of treatment of TB/HIV patients in a referral center in Iran National Research Institute of Tuberculosis and Lung Disease

Abstract

Background:Tuberculosis is an important opportunistic infection in HIV patients. It affects more than half of all untreated HIV patients.
Method: During Jan 2002- March 2004 all patients with TB/HIV coinfection were included. Data were gathered prospectively and analyzed by SPSS.
Results: 37 patients were included. Mean age was 38.2 ±7.9. Mean CD4 count was 175 ± 123. Male were predominant (89.2%).Smear was positive in 78.4% and culture in 63.6% of patients.
TB was pulmonary, extra pulmonary and both type in 67.6%, 18.9% and 13.5% respectively. 61% of patients were drug susceptible on antibiogram but 27.8% were nonresistant and 11.1% were MDR. Cure rate was 70.3% and 10.8% had treatment failure. In hospital mortality rate was 18.9%. Drug induced hepatitis developed in 16.2% of patients.
Nine of patients received HAART concomitant with anti TB.
Conclusion: Cure rate in our center is acceptable despite of advanced immeno deficiency of patients but MDR- TB is emerging as a major problem in TB/HIV patients.
