Abstract

Setting: Anti-TB Dispensary # 8, St Petersburg, Russia
Objectives: To analyse adherence to treatment after implementation of food-packages as incentive in homeless TB patients.
Study design: Case histories were analysed using standardised questionnaires and the records were organised in data base EpiInfo. An interruption of treatment was designated as an absence of >= 3 consecutive days. As controls, homeless TB patients before implemenation of incentives were used.
Results: Only 33% of the homeless patients completed treatment before Dec 1, 2001. 198 homeless TB patients were enrolled in the incentive program Dec 2001 to Dec 2003. Approx. 10% were females. 65% of all patients were former prisoners and 60% were diagnosed with alcohol/drug abuse. 80% of the homeless TB patients did not interrupt treatment. Additionally 21 homeless patients were interviewed and the most appreciated products were butter, canned meat and bread. They acknowledged the dispensary staff as important to retain patients within TB treatment.
Conclusion: Food packages as incentive increase adherence to treatment in the study population, i.e. homeless TB patients, and attract homeless to seek medical attention for TB.
