Abstract

CIIS is a scoring system based on features of patient‘s electrocardiogram that can reliably detect IHD. Furthermore it has been recently validated as a non-invasive marker that predicts mortality in COPD populations in whom IHD is prevalent but underdiagnosed. We studied 85 patients hospitalised for COPD exacerbation to our DGH looking at patient CIIS vs FEV1 and prescription of cardio-protective medications. All ECGs were independently reviewed. Degree of airflow obstruction, exacerbation frequency and length of stay were all associated with a higher admission CIIS. Only a minority of patients with high CIIS had a diagnosis of IHD in their notes. Prognostically beneficial cardiac medications did not significantly change in high scoring patients at 3 months post discharge. Our study shows an inverse relationship between FEV1 and CIIS, suggesting a need to perform this relatively simple score in all COPD patients leading to appropriate management in those with high scores. Future studies may determine whether cardio protective therapy given on the basis of a high CIIS confers survival benefit.
