Abstract

Background
As of this moment, around 28000 cases of influenza have been documented in the Republic of Moldova (3,8 million population). 6548 patients were tested for 2009 H1N1 influenza by RT-PCR, and 2707 were positive. In the same period of time there were 17824 patients diagnosed with pneumonia. Many of the most severe cases were referred to our tertiary care institution from around the country.
Objective
To describe the clinical characteristics of 74 patients with pneumonia associated with 2009 H1N1 influenza admitted to our facility from November 2009 to January 2010
Methods
We observed clinical, laboratory and radiological findings in 74 patients with pneumonia associated with 2009 H1N1 influenza hospitalized in our institution from November 2009 to January 2010.
Results
All 74 patients were admitted to the ICU and 16 (21,6%) died. The mean age was 35,3 years, 62 (86,1%) patients were female and 24 (7,3%) patients had obesity. Comorbidities were present in 34 (43,2%) cases, 24 patients were pregnant. The mean durations of hospitalizations was 14 days, and the mean ICU stay was 7,4 days. 46 patients developed ARDS, and 24 required invasive mechanical ventilation. Bilateral infiltrates on chest X-ray were present in 83,7%. Chest CT (performed only in 20 patients) showed airspace consolidation and ground glass opacity in a multilobar and bilateral distribution. S. viridans was the most frequent agent isolated in cultures of sputum samples (26/40). Secondary superinfection with P. aeruginosa was documented in 15 patients.
Conclusions
Influenza 2009 H1N1 can cause severe illness including pneumonia and death. S. viridans was the most frequent documented bacterial agent, followed by isolated P. aeruginosa.
