A role of tiotropium in patients with severe COPD subject to a supervised program of exercises

Abstract

INTRODUCTION: Several studies have demonstrated the therapeutic efficacy of tiotropium in the treatment of COPD. AIM: To analyze whether tiotropium enhances de effects of exercise training in COPD patients. DESIGN: 12 week randomized, double – blind. METHODS: 18 severe COPD (stage III/GOLD) patients were randomly divided into control group [CG] (FEV1 = 38.5 ± 1.1, N = 9), and the experimental group [EG] (FEV1 = 37.2 ± 0.7, N = 9), received 18mcg of tiotropium bromide as a single dose in the morning. Lung function, respiratory muscle strength, exercise capacity, dyspnea and quality of life were analyzed by spirometry and whole body plethysmography, simple occlusion test, by an incremental cycle ergometer test, at BDI and the SF-36, respectively. RESULTS: We observed a significant increase in MIP (- cmH2O) in both groups (CG: 65.0 ± 10 to 80.0 ± 5, p < 0,05; EG: 68.0 ± 6 to 85.0 ± 5, p < 0,05) and a significant increase in FEV1 (CG: 38.5 ± 2 % to 37 ± 4 %, NS; EG: 37.2 ± 4% to 41 ± 3 % p < 0,05) and RV / TLC,% (CG 63.5 ± 4 % to 61.0 ± 1 %, NS; EG: 65.0 ± 3% to 54.0 ± 3% p<0,005) in the EG. We also observed a significant increase in exercise capacity [Wmáx, W](CG: 43 ± 14 to 51 ± 17 W, p < 0,05; EG: 44 ± 16 to 57 ± 13 W p<0,001) in both groups with greater variation [Δ% W] in the EG (p <0.05). Moreover, we note a reduction in dyspnea (p <0.05) accompanied by a significant improvement in quality of life (CG: 32,78 ± 0,75 to 39,09 ± 7,01 NS; EG: 34,78 ± 2,75 to 59,09 ± 7,01, p<0,05) in the EG. CONCLUSION: Our results suggest that the tiotropium optimizes the results of an exercise program for patients with severe COPD.
