Abstract

Aim: Human infection with the novel H1N1 influenza virus, initially popularly termed as "swine flu," was first reported in April 2009. We aimed to assess baseline characteristics and treatment outcomes of consecutive 2009 influenza A(H1N1) patients who were followed-up in department of chest diseases without requiring mechanical ventilation.
Methods: Observational study of 24 patients with 2009 influenza A(H1N1) at ICU. Demographic data, symptoms, comorbid conditions, and clinical outcomes were collected using a case report form. Patient‘s descriptive data was evaluated.
Results: Twenty-four patients were admitted to hospital with confirmed (n=8), probable (n=6) or suspected 2009 influenza A(H1N1). The mean age of study patients was 41 (range, 18-74) years and 54.2% were female. No patient had received an influenza vaccination in 2008 or 2009. 16 patients (66.7%) had a comorbid conditions. The median number of comorbid conditions was 1 (IQR, 0-4). Asthma (20.8%) was the most common comorbidity. Myalgia (79.2%) was the most common symptom. Patients developed first symptoms at a median of 6 (IQR, 2-15) days prior to hospitalization. On the first day of hospital, the mean respiratory rate was 22.3±2.9 and mean lymphocyte count was 1.07±0.6 kU/L. Chest x-rays revealed bilateral pneumonic infiltrations in 19 (79.1%) patients. Median duration of neuraminidase inhibitor treatment was 7.0 (IQR, 5-10) days. Median duration of hospital stay was 7.0 (IQR, 4-14) days.
Conclusion: Critical illness from 2009 influenza A(H1N1) in ICU occurred in young and female individuals, and patients developed first symptoms at a very long time prior to hospitalization.
