Determinants of exhaled NO in cleaning workers: A case-control study

Abstract

Cleaning workers are at increased risk of asthma but the effect mechanisms including the role of airways inflammation need to be clarified. We studied the level of fractional exhaled nitric oxide (FeNO) in relation to asthma, atopy and other factors in cleaning professionals. A case-control study in cleaning workers included 43 cases with current asthma symptoms, and 51 controls with no history of asthma and without current asthma symptoms. FeNO was measured using the NIOX MINO® system. Forced spirometry testing was performed and atopic status was determined using the Phadiatop test in blood serum samples. Determinants of log-transformed FeNO were evaluated using multivariable linear regression analyses. Cases were on average younger than controls (43 vs. 48 years), more likely to be current smoker (49% vs. 15%) and more often had atopy (41% vs. 11%). Geometric mean (GM) FeNO concentrations for cases and controls were 18 and 20 ppb, respectively; GM ratio 0.9 (95% Confidence Interval (CI) 0.7-1.1). Atopy was positively associated with FeNO in both cases (GM ratio 1.4; CI 1.0-2.0) and controls (1.9; 1.3-2.6). Current smoking strongly reduced FeNO in cases (GM ratio 0.6; CI 0.4-0.8) but not in controls (0.9; 0.6-1.2). No significant associations between FeNO and sex, height or lung function were observed. When taking into account all determinants, the adjusted GM ratio between cases and controls was 1.0 (CI 0.8-1.2). The fact that FeNO is not different between asthmatic and non-asthmatic cleaning workers, independent of differences in age, smoking status and atopy, suggests that eosinophilic airways inflammation is unlikely to play an important role in asthma among cleaning workers.
