Responsiveness of a 3-min shuttle walking exercise test to assess dyspnea in COPD

Abstract

Introduction: Exertional dyspnea is an elusive symptom. We recently developed and validated a 3-min constant pace shuttle walking test to assess exertional dyspnea in the primary care setting (Perrault et al. COPD 2009). Aims: This placebo-controlled, double-blind study aims to evaluate the responsiveness of the 3-min constant pace shuttle walk test to detect improvements in exertional dyspnea following acute bronchodilation in patients with COPD. Methods: This study was a randomized, controlled trial with a two-period cross-over design. GOLD stage II-III patients scored dyspnea on a Borg scale during a 3-min constant pace shuttle walking exercise protocol. Tests were preceded by the nebulization of placebo or ipratropium bromide (IB) 500µg. The order in which treatment was given to each participant was randomized. Patients completed one test on each testing day. Data was collected at baseline and during exercise at 1, 2, 2.5 and 3 min. Results: Eighteen patients have completed the study. Mean age was 64 ± 5 years. Baseline pulmonary function was similar across the testing days. IB achieved significant level of bronchodilation (ΔFEV1 = 0.18 ± 0.07L, p <0.01). In comparison to placebo, there was a statistically significant reduction in Borg dyspnea score with IB during the walking test at 2 min (Δ dyspnea = 0.56 ± 1.1, p <0.05), 2.5 min (Δ dyspnea = 0.53 ± 1.6, p <0.05) and 3 min (Δ dyspnea = 0.39 ± 0.1.8, p <0.05). Conclusion: This externally paced 3-min walking protocol was responsive to detect reductions in dyspnea following acute bronchodilation in COPD patients.
