Karnofsky index as predictor of exacerbations in moderate and severe COPD patients

Abstract

INTRODUCTION:
Karnofsky index has been used to measure the clinical and functional status in hospitalized patients. The main topic of this study is to assess if this index determines the number of admissions, emergency visits, and mortality in a short and medium term, in patients with moderate and severe COPD (classified by GOLD).
METHODS:
52 moderate or severe COPD patients were included and monitored during 3 months. The variables studied were: age, sex, hospitalization days, tobacco consum, number of emergency visits, hospital readmissions and NIMV treatment.
RESULTS:
The 85% who have been studied were men and 15% were women. The averge men age was 69 years and 78 for women. The average hospitalization days was 12,5 (range 3-72). 61% were diagnosed as moderate COPD and the 39% were diagnosed as severe COPD. 42% obtained more than 60 points in Karnofsky index and 58% less than 60. Considering the smoking habits 60% were active smokers and 30% had been smoker previously. The overage index packages was 43±36 per year. 34% re-entered at the Pneumology Department at least once for COPD exacerbation, and 41% visited Urgencies at least once for the same reason. There was a 17% of mortality during their time at hospital and the following three months.
There are stadistical differences between a hirgher mark than 60 points in Karnofsky Index and the number of visits at Emergences and readdmitions for COPD exacerbations. The patients who had a Karnofsk Index higher than 60%, suffered a higher mortality than the rest.
CONCLUSIONS:
Karnofsky index is a good prognostic predictor of mortality, hospitalizations and emergency visits for exacerbation for severe and moderate COPD patients.
