Test-retest reliability for the 30-meter walk test (30mWT) in patients with chronic obstructive pulmonary disease (COPD)

Abstract Background: Chronic obstructive pulmonary disease (COPD) has significant extra pulmonary effects that affect physical function. Having reliable, non-complicated and low-cost tests to measure physical function is crucial if tests are to be implemented in clinical practice. Aims and objectives: The main objective was to examine test-retest reliability for the 30mWT. A secondary objective was to compare the 30mWT to the 6-min walk test (6MWT). Methods: Forty-seven patients with stable COPD were included from two university hospitals in Sweden. The 30mWT, which consists of walking in self-selected speed and after 2 minutes of rest walking in maximal speed, was conducted twice with 7-14 days in between tests. The 6MWT was performed in duplicate the same day. Results: Test-retest reliability for the 30mWT was high, r = 0.95 (p<0.0001), and intraclass correlation coefficient (ICC) = 0.93 (95% CI: 0.87 – 0.97) in maximal walk speed versus r = 0.87 (p<0.0001), and ICC = 0.87 (95% CI: 0.78 – 0.93) in self-selected walk speed. The correlation between maximal walk speed in the 30mWT and distance walked in the 6MWT was r = 0.78 (p<0.0001), and the correlation between self selected 30m walk speed and the calculated walk speed of the 6MWT was r = 0.73 (p<0.0001). Conclusion: The 30mWT is a reliable sub maximal test which is time-saving and easy to perform. It might be used to measure physical function in patients with COPD. Presumably it should be performed at maximal walking speed because of a better test-retest correlation and better association to the 6MWT.
