Impact of cough-related quality of life on fatigue and depression in chronic obstructive pulmonary disease

Abstract

Aim: One of the most common symptom in chronic obstructive pulmonary disease (COPD) is persistent cough. The aim of this study was to assess the relationship between cough symptom, fatigue and depression in clinically stable COPD patients.
Methods: Twenty-eight COPD patients (60.64±8.67 years, FEV1: 53.68±24.93%) were included in study. Cough symptom was evaluated using Leicester Cough Questionnaire (LCQ). The LCQ is a symptom specific questionnaire designed to assess the impact of cough severity with physical, social and psychological domains. Actual fatigue was evaulated with sit-ups for peripheral endurance assessment. Perceived fatigue was assessed using Fatigue Impact Scale (FIS). Depression was measured using Beck Depression Inventory (BDI).
Results: Physical, social and psychological domains of LCQ were significantly correlated with the mean number of sit-ups performed (r=0.72, r=0.69, r=0.61, respectively), FIS score (r=-0.59, r=-0.57, r=-0.48, respectively), and BDI score (r=-0.55, r=-0.52, r=-0.45, respectively, p<0.05). The LCQ total score was significantly related with the mean number of sit-ups (r=0.70, p<0.0001), FIS score (r=-0.55, p=0.003), and BDI score (r=-0.53, p=0.005).
Conclusion: Chronic cough has negative effects on abdominal muscle endurance, fatigue, quality of life and depression. Therefore, improvement of abdominal muscle endurance has impacts on perception of fatigue, cough-related quality of life, and psychosocial status in patients with COPD.
