Heterogeneity of regional lung ventilation in COPD patients determined by electrical impedance tomography

Abstract Introduction: Unlike conventional lung function testing, electrical impedance tomography (EIT) is able to assess not only global but also regional lung function. The aim of our study was to determine the heterogeneity of lung function in COPD patients using EIT.Patients and Methods: 28 COPD patients (GOLD II-IV) were studied. EIT and spirometry examinations were performed in parallel before and 5, 10 and 20 min after inhalation of salbutamol or ipratropium/fenoterol. EIT data were collected at 44 scans/s (Goe-MF II, CareFusion,Höchberg,Germany). FVC, FEV1 , FEV1 /FVC, inspiratory vital capacity (IVC), residual (RV) and tidal volumes (VT ) were determined by EIT in the whole chest cross-section in 912 image pixels. Statistical analysis was carried out by ANOVA and Bonferroni test.Results: 3 patients did not complete the study because of respiratory exhaustion or technical problems. No bronchodilator effects were found in 12 patients (67±10 yr, mean±SD). 13 patients (74±6 yr) showed a positive bronchodilator effect. In the first group, EIT also did not detect any significant effect of bronchodilation on the studied measures, neither on global nor regional level. However, highly significant differences in regional variance of FVC, IVC, RV and VT were found between the ventral and dorsal regions which were not affected by bronchodilation. In the second group, a significant increase in FVC was detected by EIT, especially in the dorsal regions. The differences in regional variance of FVC, IVC, RV and VT were less pronounced and further reduced by bronchodilation.Conclusion: EIT is able to detect regional lung function heterogeneity in COPD patients and the effects of bronchodilation.
