Asthma treatment in postmenopausal women in Ukraine: what HRT adds?

Abstract

With the goal to assess influence of hormone replacement therapy (HRT) on treatment results 167 female patients with mild-to-moderate asthma were selected, 81 of which agreed to receive HRT during the last 2 years (HRT group). The results showed significantly better level of asthma control in HRT group (as compared to the group without HRT) with less attacks (0.14 vs 0.37 per night, p<0.05), lower daily dose of beta-agonists (123.7 vs 189.9 mcg/day, p<0.05), better FEV1 (89.7% vs 81.3% of predicted, p<0.05), higher PEF during the last two weeks (85.4 % vs 68.7% of personal best, p<0.05). With the background that there is no re-imbursement in Ukraine and the majority of patients prefer mostly life-saving drugs and relievers we studied also the compliance in both groups with a specifically designed questionnaire, scaled from 0 to 10. The HRT group showed significantly better compliance (7.7 vs 4.3, p<0.01) which can be explained that those patients who adhere to preventive treatment like HRT were much more concerned about their general health condition as well as asthma. We conclude that: 1) Women who receive HRT have significantly better level of asthma control at the same time showing also higher level of compliance to treatment; 2) Conclusion of HRT efficacy in asthma patients in Ukraine should allow for patients compliance; 3) Such cross-influence of HRT and asthma treatment may be characteristic mostly to countries with lower income and insufficient re-imbursement system.
