Abstract

Introduction
Adherence to pulmonary rehabilitation (PR) in the UK is known to be suboptimal. Reasons for this have been explored, but there is still limited understanding of the patient experience of pulmonary rehabilitation in the UK. This study aimed to improve our understanding of the experience of patients with COPD, and their perceptions and beliefs about PR, prior to attending a programme.
Method
Participants were recruited from the PR waiting lists from two UK NHS Trusts and interviewed individually before their initial assessment into a PR programme. Interviews were digitally recorded, transcribed verbatim and analysed using a phenomenological approach.
Results
Twenty-three participants aged 48 to 90 (14 males) were recruited. Common themes have emerged which have increased our understanding of the patient experience. They include: the importance of living with co-morbidities; the close experiential relationship between feeling breathless, feeling frustrated or anxious, and feeling panic; the experience of confusion regarding the care of COPD; the view of PR as something to try with a sense of hope.
Conclusion
This study has shed light on the experiences of COPD patients prior to entering pulmonary rehabilitation and suggests that many approach PR with positive feelings i.e. hope and expectation. This would suggest that subsequent non-adherence to PR may be related to a dissonance between patients‘ hopes and their subsequent experience, but further research investigating perceptions after the PR programme is required to explore this possibility.
