Cytokines and alpha-1proteinase inhibitor (A1 PI) levels in meat wrappers‘ asthma

Abstract

The heating of plastic packaging film is known as a cause of meat wrapper‘s (MW) asthma. However the underlying mechanism of it disease is not clarified yet.
This study aims to explore the effects of exposure to polyvinilchloride (PVC) products on the airways and cytokine profile in MW with different A1PI genotypes.
Methods: 45 MW and 77 workers of another trades in meat processing and packing factory, both male and female, the overall mean age 40,1 yrs, mean length of service 11,7 yrs were examined at the work place during the shift. All subjects were underwent to questionnaire, peak expiratory flow (PEF), spirometry, blood cytokines levels (IL-4, IL-6, IL-8, TNF-α) measurements and genotypes analysis of A1PI.
Results: 18 out of 45 (40%) MW had a higher prevalence of cough, hay fever, breathlessness, compared to other workers 19 of 77 (15,6%) P<0.05. The risk of asthma symptoms, as well as decreases in FEV1and appearence of airway hyperresponsiveness after the shift (68.4±8.2 vs 89.8±6.0, P<0.01) was greater in MW with an adjusted odds ratio (OR) of 7.1 (95% CI, 1.9-54.5). Asthma symptoms were followed by increased expression of IL-6, IL-8, TNF-α, reduction of IL-4 in correlation with FEV1, FEV1/FVC changes (r1=0.6, r2=0.4) and the presence of A1PI deficiency (PISS, PIMZ genotypes).
Conclusion:Our data suggest that exposure to PVC compounds can lead to asthma and immune abnormalities in MW with PISS and PIMZ genotypes mainly.
