Is the decrease of AVR (arterovenous ratio)in patients with severe OSAS, reversible after long term use of CPAP?

Abstract

A decrease of arterovenous ratio (AVR) is related to an increased risk of cardiovascular and cerebrovascular disease,in OSAS patients nocturnal hypoxemia can induce retina vessel constriction and occlusive disease of carotid arteries . The aim of the study was to evaluate in OSAS patient the AVR to find a correlation with severity of apnea/hypopnea index(AHI) and AVR values. 20 males (M)and 10 women(W) Aged from 35 to 60 y.o , no smokers no diabetics no heart diseases, obese (BMI=35+18kg/m2)diagnosed OSAS after a polisomnography, underwent retinography of two eyes with Topcon TCR NW200 non midryathic retinal camera. specifically planned for the screening the retinopathy.After 8-hr abstention from drinking coffee or beta stimulant drugs, each patient underwent retinal photography in order to measure vessel diameters.The static retinal vessel analisis was performed in :10 (W) AHI=15,5±0,5(mild OSAS) and 20 (M) AHI=50,5±7(severe OSAS)In the group of severe OSAS ,AVR = 0,75±0,1 pathological constrincted,in the mild OSAS group AVR=0,84±0,009(normal) .The lower AVR is related with AHI severity and statistically significant p=0.001 (value calculates on variance of AVR.) .The control of AVR in group of patient (M)with severe OSAS 1year after the use of nocturnal CPAP,was not statistically modified(AVR basal=0,75±0,1 vs 1 years after AVR =0,76±0,4(n.s) , but the ultrasound examination of the carotid and vertebral arteries showed a presence of occlusive diseases of carotid arteries .In conclusion the AVR index, in severe Osas patients is not reversible after long term nocturnal CPAP treatment,but can is a earlier signe of occlusive disease of carotid arteries
