Statins and outcome of COPD patients after hospitalization for COPD exacerbations

Abstract

Background:Retrospective studies have shown that treatment with statins is associated with improved survival after COPD exacerbations (ECOPD).
Objectives:To evaluate prospectively the role of statins on the outcomes of patients hospitalized for ECOPD.
Methods:We studied prospectively 245 patients admitted to hospital for ECOPD. Patients‘ characteristics, COPD severity, treatment and health-related quality of life (HRQoL) were recorded. Patients were evaluated monthly for one year recording new ECOPD, and survival HRQoL was recorded at the time of hospital discharge, and two, six and twelve months later. Patients whose statin therapy was interrupted for any reason were excluded from the study. Decisions for treatment and discharge were made by attending physicians not involved in this study. The role of statins on outcomes was evaluated by Cox regression analysis after proper adjustments.
Results:Patients receiving statins as outpatients prior to their admission had better survival from ECOPD compared to patients not receiving statins (p<0.001). The use of statins for one year after hospital discharge did not improve survival (p=0.681), but patients on statins had less ECOPD in one year compared to patients not receiving statins (2.1±2.7 vs. 2.8± 3.2 respectively, p=0.037). Moreover, the use of statins prolonged time to next ECOPD and next severe ECOPD (p<0.001 and p=0.015 respectively). Finally, the use of statins was related to a better improvement in HRQoL at 2, 6 and 12 months after discharge (p<0.001).
Conclusions:The use of statins does not affect one-year survival after ECOPD, but is related to a smaller number of ECOPD and prolonged time to next ECOPD, as well as better improvement in HRQoL.
