Sleep-disordered breathing in patients with chronic heart failure: Prevalence and sleep characteristics

Abstract Background: Sleep-disordered breathing (SDB) is common in patients with chronic heart failure (CHF) and influences the progression of the disease. We report on the multi-centre SchlaHF registry in German CHF patient.Methods: In an ongoing prospective multi-centre registry (n=4,252) so far 1157 CHF patients underwent a polysomnography (PSG). New York Heart Association (NYHA) class was ≥II and left-ventricular ejection fraction (LVEF) ≤45%. Patients were screened with a two-channel device and referred to a sleep laboratory in case of suspected SDB. Using PSG we studied SDB prevalence and sleep characteristics in these referred CHF patients. SDB was defined as apnea-hypopnea index (AHI) >15/h.Results: In 41.0 % of the patients we found central sleep apnea (CSA), in 34.6% obstructive sleep apnea (OSA) and in 24.4% no SDB. Sleep efficiency was not significantly different (CSA 75.5%, OSA 74.2%, no SDB 74.5%). Sleep duration in S1 was in CSA 28.7 %, in OSA 24.4 % and in patients without SDB 19.7 % (p<0.05 for all). Sleep duration in S2 was in CSA 44.4%, in OSA 47.4% and in patients without SDB 44.8% (non-significant). Sleep duration in S3/S4 was in CSA 10.4%, in OSA 11.8% and in patients without SDB 14.6% (OSA and CSA significantly different to patients without SDB). Sleep duration in REM was in CSA 10.3%, in OSA 11.5% and in patients without SDB 12.3% (CSA significantly different to patients without SDB).Conclusions: High prevalence of CSA and OSA in HF patients is confirmed compared to recent studies. In general, in the SchlaHF registry sleep duration and sleep efficiency was similar in CSA, OSA and in patients without SDB.
