Abstract
Rationale:
Fat-free mass (FFM) has been related to muscle function and exercise intolerance in patients with COPD but has not been used as a descriptive characteristic in exercise studies. We investigated the efficacy of two training methods in FFM-depleted COPD patients.
Methods:
31 depleted COPD patients were randomly assigned to traditional whole-body endurance and resistance training

32.8 ± 9.5%pred, FFM-index 14.0 ± 1.2 kg/m

) or individualized resistance training (IRT; N=15, 60.2±7.5 yr, FEV

27.8 ± 7.7%pred, FFM-index 13.4 ± 1.2 kg/m
2
) with distinct protocols to induce different physiological adaptations. Training was performed (3 days/wk) for 6 weeks. Endurance was assessed with a cycle test (CWT) at 75%Wmax.
Results:
An increase in CWT was found in both groups: ERT (287 ± 217.7 to 372.8 ± 264.9 sec; p<0.01) and IRT (318.1 ± 198.6 to 702.1 ± 384.5 sec; p<0.001). We found a larger increase in IRT compared to ERT (376.5 ± 312.6 vs 85.4 ± 115.2, p<0.01)
Conclusion:
Our results show that FFM-depleted COPD patients profit more from individualized resistance training. These findings should be incorporated in training strategies for patients with COPD.
