Mean platelet volume in patients with obstructive sleep apnea syndrome

Abstract Epidemiologic data support the concept that obstructive sleep apnea syndrome (OSAS) is an independent risk factor for cardiovascular diseases. These observations have prompted investigations of the role of OSA in cardiovascular disease pathogenesis. Platelets play a key role in ischemic cardiovascular disease and increase in platelet aggregability and activation has been demonstrated in patients with OSA. Mean Platelet Volume (MPV) is a marker of functions and activities of platelets. Thrombocyte activity and aggregation potential can be conveniently estimated by measuring mean platelet volume (MPV) as part of whole blood count. The aim of this study was to investigate the relationship between MPV and features of OSA and the level of MPV in OSA. 138 consecutive patients (97 OSA, 41 non-OSA) admitted to Duzce University School of Medicine Sleep Breathing Disorders Laboratory from 2006 to 2009 have been included in this study. Patient with Apnea Hypopnea Index over 5 has been accepted as OSA (n:97). Those below 5 have been accepted as control group. MPV has been measured Sysmex-xt-2000i CBC device. The MPV in OSAS (mean±SD, 10.03±1.33 fL) and control group (mean±SD, 10.22±1.14 fL) was similar. The MPV in clinically significant OSAS (AHI ≥ 15, n=65) was significantly lower than that of mild OSAS (AHI < 15 n=73). (respectively, mean±SD, 9.85±1.35 fL, 10.29±1.17fL, p=0.045). As AHI and desaturation index increases, the MPV decreased significantly (respectively, p=0.047 r=-0.170, p=0.004 r=-0.251). As the severity of OSAS increases, MPV decreases. Current findings contrary to expectations, platelet function and activity in patients with OSAS seem to be decreased. Further studies are needed to evaluate this relation.
