Bradykinin receptor gene polymorphism and quadriceps strength in COPD

Abstract

Skeletal muscle depletion is a significant complication affecting patients with chronic obstructive pulmonary disease which has a complex aetiology. Quadriceps strength in these patients has been shown to be associated with angiotensin converting enzyme (ACE) gene polymorphism. We hypothesized that might therefore be a link between quadriceps strength and the (+9/-9) polymorphism of the bradykinin receptor (BK2R) since this has previously been shown to influence metabolic exercise efficiency and left ventricular hypertrophy in response to training. We studied 110 patients with a mean(SD) FEV1 34.3(16.4)%predicted and 104 healthy age-matched controls. In patients who were homozygotes for the +9 allele (n=37), which is associated with lower receptor levels, isometric quadriceps strength was significantly lower 30.8(10.4)kg vs 36.4(12.8)kg p=0.02, as was fat free mass index; 15.7(1.8)kg.m-2 vs 16.6(2.3)kg.m-2 (p=0.04). There was no evidence of an interaction with ACE genotype. Bradykinin receptor genotype did not influence any parameter measured in the control group. A higher level of the receptor appears to preserve fat free mass and strength in patients with chronic obstructive pulmonary disease.
