Aims: To describe a diagnostic and therapeutic strategy for patients with persistent symptoms after acute pulmonary embolism (PE); to discuss the epidemiology of symptoms after acute PE; the differential diagnosis; the pathophysiological mechanisms of disease, including dead space ventilation, right ventricle dysfunction, and pulmonary hypertension; an overview of how to manage patients with persistent symptoms; and current treatment strategies including pulmonary endarterectomy (PET) and balloon pulmonary angioplasty (BPA).
