This is actually an outstanding presentation, quite challenging, addressing very important issues for researchers and clinicians. It clearly deserves a most prominent position among the ERS e-lectures and I strongly recommend it as a teaching material to foster discussion on this interesting topic with students, residents and senior staff. Although, incidentally, I do not fully agree with some of the recommendations of the Task Force, but … I was very happy to be able to hear their explanations and to be able to make some questions.

Very interesting in a difficult theme thoug very frequent in the clinic. I would sugest to do a big multicenter study trying to clarify all the questions raised. Very good job for all in the task force, and keep going.
