Aims: To revive the interest to clarify the underlying mechanisms of airway hyperresponsiveness (AHR). AHR is a risk factor for both asthma and COPD, and is central to the diagnosis and severity of asthma, and to disease progression and mortality in COPD. The same phenotype of AHR may have different underlying mechanisms in asthma and COPD, but how and why?
