Aims: To describe the pathophysiological background of right heart catheterisation (RHC) and provide a full comprehensive update on the use of RHC for studying the pulmonary circulation; to describe practical issues regarding the assessment and interpretation of exercise pulmonary haemodynamics with a focus on the reliability and correct interpretation of measurements taken during exercise, zeroing, respiratory swings, normal responses, and different pathological patterns; to discuss from a practical point of view the role of non-invasive methods in assessing exercise pulmonary haemodynamics; and to incorporate the pulmonary arterial hypertension patient's perspective.
