Aims: An understanding of what integrated care means and how the component parts of integrated care are identified and fit within a pathway of care. The session will also identify the organisation and delivery of care at the time of an exacerbation, how technology can be considered to enhance care and finally the importance of primary care in the delivery and co-ordination of care for these patients.
